    She is right about insecure docs being confronted by the knowledgeable with medical terminology. And that was prior to the google age. Nowadays many if not most come armed (harmed?) by bits they pick up online, which confounds both patient and doc. Half our effort appears correcting misconceptions and sometimes malignancy. Most careful generalists spend the balance of their office visits educating educating educating…

    Regarding medical rackets in the fallen West.. I read somewhere a military surgeon returning from Crimea (1856!) to France and saying the entire system was built for profit even then.     That doesn’t sound good. A quarter-century old foreign body (FB) which may be impacting a high-pressure artery.. these FBs are the dangest things as you know. Can stay dormant for decades then cause hell. But this may not be a FB issue at all.

    A decision tree would be rapid work-up to ensure this is not consequential. Better to get surgeries done sooner rather than later… The pain and throbbing may due to another condition, a likely arthritis. The FB could be incidental. engorgement similarly may be misleading and due to other effects. Is it his dominant arm? Manual workers, like athletes, can exhibit features of strong one-sided use.

    Sonography, and possibly catheterization / endoscopy, nerve tests, etc… would determine next steps and whether surgery is indicated at all. If can afford, take duplicate tests at several unrelated hospital systems (and similar for second opinions of surgeons). Mercenary California! Keep in mind trustworthiness. I always had a soft spot for the hippies from Oregon; I had a roommate once from Corvalis, and his name was “Sky!”

    PS – I am like Saker, I don’t believe in races or racism. It’s a nebulous term. But I do think most humans on every continent are ignorant, xenophobic and love to hate those who’re different, lol. It is the lucky one, or highly educated, that realises it’s ridiculous waste of time.     US resumes biolabs program in Ukraine โ€“ Russian MOD

    “A new trove of documents on alleged US-funded biological programs in Ukraine was presented by the commander of Russiaโ€™s Nuclear, Biological and Chemical Defense Forces, Lieutenant General Igor Kirillov, during a media briefing on Friday.

    Talking of pipes and sewers and construction, as you know until the relatively recent advent of anesthesia, the surgical field was unfairly disparaged. Not even considered a medical field! Openly mocked. Consigned to being barbers on the side or sometimes medieval executors of state will to make ends meet. I really chortled as a teen reading about the terrors ordinary sailors had of them on Pirate Ships (pre-anesthetic Napoleanic wars of British RN) โ€” iirc called sawbones and worse; the Angel of Death and Davy Jone’s Locker had better reputations.

    Even two decades ago my generalist attendings called them primitive “Plumbers.” They are the most sued docs in litigation-happy USA, due to their [lack] of communication skills and perceived arrogance… But they get the last laugh in this, their Golden Age. So many tools, toys, and advancements at their disposal, esp with visualization at the micro level. They can really achieve wonders for the right patient with the right procedure, that’s for sure. It is a genuine pleasure to see the turnaround as they’ve benefited so many.

    But care must be taken to weigh the pros and cons, given their gung-ho inclinations, surgical complication rates, and even the very necessity of much they do. So much is elective nowadays, due to vanity or obsession of meeting unrealizable ideal. A fitting punishment for Exceptional minds and attitudes, no?! Surgery in all traditional societies, regardless of need, was always considered the last option for simple reason it is usually irreversible or comes at potentially catastrophic cost exceeding the driving health problem. And so many are impatient and rush to them.

    Nowadays, we have tremendous advancements in physical therapy and rehab, rediscovered herbals and alternative med, and other modalities that must be first exhausted.

    Are your wrists and arthritis acting up? <span style=”text-decoration: underline;”>It isย unavoidable as we age</span>. Especially those engaging in strenuous hand labor (and the occasional bike spill), both of which you qualify! One of my favorite cynic lecturers (a pediatric cardiologist) remarked atherosclerosis starts in utero โ€” i.e. we start dying even prior to birth! The natural plumbing wears away and atrophies and calcifies as we age. <span style=”text-decoration: underline;”>Consider carpal tunnel syndrome (CTS)</span> as we’re all sitting in front of screens so much that it has become ubiquitous. That is median nerve entrapment which if it advances and becomes intractable requires decompression surgery. As long as function is not lost and pain is manageable I recommend taking care given untold new risks on top of Imperial collapse changing the health care landscape.

    Presurgical options:

    • Can you power on your grandfather’s old effectiveย gizmos (ozone?)? At essence, these are issues of insufficient circulation.
    • Try rehab and exercises to make the muscles limber, increase blood circulation, lower inflammation, increase the anastomoses of the ulnar with the radial blood supply, etc. If one bodyย area is damaged, a trick is to compensate with strengthening and acquiring support from the adjacent parts. Squeeze a tennis ball repetitively. Visit a physical therapist (PTOT) often working with rehab medicine in hospitals/clinics. They are my favorite allied health and do wonders, often obviating surgery. They will assess and advise on take-home exercises and protective measures.
    • It may be insidious Carpal Tunnel on top of preexisting arthritis.. observe ergonomic seating and arm positions which do wonders to protect the wrists, hands, and forearms. If they tingle or ache soon after being on the computer, you need to sit right. Often elevating our seats, adjusting table height (or changing altogether), or somehow lowering the arms does the trick.. the key is maintenance of 90 degrees at elbow joint, so that the wrist does not need to pull up and backwards toward forearm.
    • There are many many herbals and natural remedies to boost circulation; acupuncture; magnets; etc etc (not my area)

    Studies:

    Fortunately most studies worldwide show there are significant and preserved anastomosis between the ulnar and radial arteries in the hand. These two between them cover most palmar arch circulation. Surgeons worldwide may have the most experience with the radial artery since it is the most preferred donor flap (called the Radial Forearm Flap (RFF)) for reconstructive surgery of most large defects throughout the body. And used a lot in coronary bypasses and instrumentalization. It has become relatively safe and effective given such experience and wide use..

    The prevailing vascular patterns helps inform surgical approaches and risk.ย Of courseย specific risk depends on the specific ailment, its source, location (deep or superficial), available expertise and abilities of surgeon, ancillary and post-OP support, age/state of patient health informing prognosis and recovery speed, etc…

    In short, these hand surgeries may be among most established surgeries. The anatomy often has good redundancy in blood supply. The challenges, risks increase with age. Always first exhaust non-surgical options. Keep in mind hyperstressed and dying US health care system as everything goes to hell! [Since you live in northern Cali, would Oregon or Idaho be safer? Mormons have excellent health care too! Pl explore]

    I strongly agree with typical corruption at construction, racist hatred of Asians, and “lit fuzes,” but Fukushima was additionally juiced along! It was such an unnatural phenomenon. It was at a weak point for Empire, and Japan was threatening vassal-escape.

    In early 2011, the US:

    • Was militarilyย mired, exhausted and attrited in unwinnable Long Wars in the greater M.E.
    • Economically weakened and kept afloat through financial dole since 2008
    • China after 1.5 centuries was returning to its usual economic and industrial preeminence
    • The entire hollowed-out hyperfinancialized & deindustrialized West was in senescence
    • Afro-Asia and even Monroe-Doctrine lands were rising up
    • A weak, effeminate optics-focused Obama was not in charge of anything, except droning on Tuesdays and how he got it from Big Mike

    So in those (and many other) circumstances, they were in a panic. They went the usual “demonstration” route and splattered Japan, to warn others too.     Mr P, there are some technical issues, pl be patient. One of my comments disappeared from two days ago and just reappeared then disappeared again! I approved your own message from few hours ago. dunno what’s happening but this is new blog and coming under increased scrutiny and attacks.

    yeah I followed fluoride for years. We spoke about it in late 2021 too iirc at the Cafe. A useful way to dumb down an entire population. Docile nutzlosen, no? I read it degrades in heat too, so even if lack osmosis, sit it in the sun if lack the tech and presto.

    The reality is everything’s been poisoned.. everything. whether hormones, radioactive products and byproducts, antibiotics, chemicals, raw sewage, toxic wastes, fracking, etc etc. Hal was just annoucning mRNA now going into livestock (= likely done years ago). Theres no escaping it, like wifi radiation. We just have to constantly detox through diet, lifestyle, mindfulness, prayer, grounding, etc etc. A good deal of our time, energy and focus will be maintenance of health for the foreseeable future. Forewarned is forearmed. To be born in our world in this age is a challenge, to put it mildly.

    Trump’s fate is curious. Yeah the biden complex do not do subtle. There was a semi-crazy armenian-californian I listened to several years ago.     We’ve beenย deprived too long of your insights. Once you get the hang, this site’sย a lot easier, and easy on the eyes too. And no topics verboten! We just miss the Poets (Anomalous! ๐Ÿคฌ) and the Russian cultural bits by Uncle Bob (and Uncle Davy, Alabama et al). Its starting to feel the Magnificent Seven (1960) or Seven Samuraiย ride again.

    PS – yes I know and agree about the incestuous relationships, that’s why I said, “useful cops.” It’s like the Brits burning their ICC tools for that frivolous “arrest warrant” of VVP; evidence of deep agony and pathologic stirrings.     Russia bans SWIFT

    “Banks have been ordered to use the countryโ€™s domestic financial messaging systems for transactions

     

    The Bank of Russia has forbidden Russian financial institutions to use the SWIFT messaging system for transactions inside the country, the regulator announced on Monday.
